Advice
- The results can be used, in combination with clinical judgment, context, and patient preferences, to create care plans and establish goals of care.
- Frailty management often requires a team-based approach, including physicians, nurses, physical and occupational therapists, dietitians, pharmacists, and social workers.
Management
Potential interventions to address frailty include (but are not limited to):
- Physical therapy and exercise programs to increase mobility, improve gait, and decrease falls.
- Occupational therapy to enhance the ability to perform activities of daily living.
- Nutrition counseling and supplementation to mitigate weight loss and augment nutritional status.
- Medication reviews with the goal of discontinuing unnecessary drugs, substituting potentially safer medications, and reducing side effects.
- Palliative care consultation to alleviate symptoms and preserve or improve quality of life.
- Case management to facilitate adequate home services or a transition to assisted living or skilled nursing facilities.
