    IMPACT Score for Outcomes in Head Injury

    Predicts 6-month mortality and unfavorable outcomes after moderate to severe traumatic brain injury (TBI).

    INSTRUCTIONS

    Use for patients ≥14 years old with GCS ≤12 (moderate and severe TBI). Use data from the first 24 hours of admission.

    Why Use

    Patients ≥14 years old admitted to critical care units with GCS ≤12 (moderate and severe TBI).

    • Developed to provide early prognostication; therefore, data from the first 24 hours of admission should be used.
    • Should NOT be used for serial evaluations or updating prognosis with progression of care.
    • Original datasets were developed in high-income countries and therefore may not be applicable in developing countries or areas with otherwise limited medical resources. 
    • Best used for isolated head injuries, as other comorbidities and extracranial injuries are not taken into consideration in this score and may contribute to patient mortality.

    Prognostic models aim to provide a spectrum of probable outcomes in patients based on populations of patients. This tool provides useful information that may help clinicians and surrogate decisionmakers to determine the goals of treatment and care relative to the possibilities of outcomes.

    Ewout W. Steyerberg, PhD, is a professor at Leiden University Medical Center and Erasmus Medical Center in the Netherlands. He is also the head of the department of medical statistics and bioinformatics. Dr. Steyerberg’s research focuses primarily on biostatistics, clinical epidemiology, and medical decision making.
