    RIFLE Criteria for Acute Kidney Injury (AKI)

    Classifies severity of acute kidney injury, similar to AKIN Classification.

    INSTRUCTIONS

    Note: the original paper refers to "acute renal failure", but the generally-accepted term currently is "acute kidney injury", which is reflected in this calculator. 

    Pearls/Pitfalls

    • Requires baseline serum creatinine, which is often not known in clinical practice. In CKD patients, baseline creatinine determined assuming GFR of 75 mL/min/1.73 m² has low correlation with actual creatinine, resulting in overestimation of AKI incidence (Lopes 2013).

    • Developed using the MDRD GFR Equation, which is validated in CKD patients with stable renal function but not validated in AKI.

    Dr. Bellomo on the RIFLE Criteria:

    We developed the RIFLE Criteria because there was a clear need for a definition of AKI. RIFLE is easy to apply. The major problem is that outside the ICU, the urinary output data are often not available. Clinically, I use it as a research tool and to understand the epidemiology of AKI in my hospital. Research in AKI is rapidly evolving and those interested in the field will be able to find our work here.

    Rinaldo Bellomo, MBBS, MD, FRACP, is a professor of intensive care medicine at the University of Melbourne and an honorary faculty of medicine at Monash University in Australia. His work in publishing over 1,300 scientific papers has earned him over 60 national and international awards and grants. Dr. Bellomo is involved in many areas of research involving intensive care, including traumatic brain injury, sepsis, transfusion, and acute lung injury.
