Advice
  • This PTS assessment tool should complement, not replace, clinical judgment. 
  • The diagnosis and severity of PTS should be discussed with the patient, taking into account their symptoms, impact on quality of life, and preferences. 
  • Some symptoms or signs (e.g., infection, malignancy, or other mimics) may resemble PTS but have different management implications; consider alternative diagnoses if clinical findings are atypical. 
  • Conservative management is often appropriate for mild to moderate cases, but more advanced interventions may be considered in severe or refractory cases.
  • Multidisciplinary input is valuable, especially for complex or severe cases.
Management
  • Mild to moderate PTS:
    • Initiate conservative measures, such as elastic compression stockings (ECS).
    • Advise on leg elevation and activity modification.
    • Educate the patient about symptoms and self-care.
  • Moderate to severe PTS (or when conservative measures are insufficient):
    • Consider intermittent pneumatic compression or advanced compression devices. 
    • Referral to a vascular specialist for assessment of endovascular or surgical options may be warranted, especially if symptoms are lifestyle-limiting or if ulcers are present. 
    • Address wound care and infection risk in patients with ulcers.
  • Refractory or severe PTS:
    • Multidisciplinary evaluation to consider endovascular or surgical interventions.
    • Shared decision-making is essential, as risks and benefits of advanced therapies should be balanced with patient preferences and comorbidities.
  • Refer to local protocols and specialty guidelines for specific recommendations on advanced treatments.
Critical Actions
  • Venous ulcers and severe PTS require prompt wound care and infection control; do not delay management in the presence of infection or signs of systemic illness. 
  • Remember that this tool is a guide; atypical presentations or comorbidities may require deviation from standard management pathways.
