    COVID-GRAM Critical Illness Risk Score

Predicts risk of critical illness in hospitalized COVID-19 patients.

    IMPORTANT

    Launched during COVID-19 crisis.

    INSTRUCTIONS

This score was developed in China and includes hospitalized inpatients and their risk of critical illness; the authors do not state the indications for admission.

    When to Use
    Pearls/Pitfalls
    Why Use

Patients admitted to the hospital for presumed COVID-19 infection.

    • This score is only for hospitalized patients and did not include ED patients; it should not be used to suggest which patients should be admitted or discharged.
    • While the study included 2300 patients, the average age of the admitted patients was quite young (48 years old). Very few had multiple comorbidities, which may make this study less applicable to other populations, like in the US.
    • For example, while the authors use a list of 10 possible comorbidities, only 3 patients had 5 comorbidities; 5 of these comorbidities would be relatively common in populations in the US.
    • The score was internally validated but has not been externally validated.

This score may help risk stratify admitted patients to help predict which should have closer monitoring (more frequent assessments or ICU admission) due to risk of progression to critical illness.
