Identifies risk of stroke in the next 7 days in patients who experienced symptoms of TIA.

Clinical Findings

First TIA in lifetime

Symptoms ≥10 mins

History of carotid stenosis

Already on antiplatelet therapy

History of gait disturbance

History of unilateral weakness

History of vertigo

Initial triage diastolic blood pressure ≥110 mmHg

Dysarthria or aphasia (history or examination)

Investigations in Emergency Department

Atrial fibrillation on ECG

Infarction (new or old) on CT

Platelet count ≥400×109/L

Glucose ≥15 mmol/L (270 mg/dL)

    • Low risk (scores ≤3): Outpatient management with a primary care physician.
    • Intermediate risk (scores 4–8): Investigations with follow-up within 2 days, including a full workup for the cause of TIA.
    • High risk (scores ≥9): Same-visit consultation and investigations.
