G8 Geriatric Screening Tool
Identifies elderly cancer patients who could benefit from comprehensive geriatric assessment (CGA).
Advice
This tool should not be used as the sole determinant for management decisions; it must be integrated with clinical judgment and a formal assessment.
Management
Score ≤14: Proceed with a CGA to evaluate specific vulnerabilities and guide interventions.
- While there is no universal CGA tool, this assessment may include (referenced tools/scores are merely suggestions):
- Assessment of chemotherapy toxicity (e.g., CRASH Score, CARG-TT).
- Estimation of (noncancer) life expectancy.
- Functional assessment (e.g., instrumental activities of daily living).
- Comorbidity review (e.g., CIRS-G).
- Fall risk screening (e.g., Get Up and Go test, BBS).
- Depression screening (e.g., GDS-15).
- Cognitive impairment screening (e.g., Mini-Cog, MoCA).
- Nutritional status assessment (e.g., weight loss, BMI).
- Please see the ASCO guidelines for additional recommendations and potential guided interventions.
Score >14: Consider proceeding with planned oncologic treatments, but remain vigilant for signs of emerging frailty or functional decline.
