Advice

This tool should not be used as the sole determinant for management decisions; it must be integrated with clinical judgment and a formal assessment.

Management

Score ≤14: Proceed with a CGA to evaluate specific vulnerabilities and guide interventions.

  • While there is no universal CGA tool, this assessment may include (referenced tools/scores are merely suggestions):
    • Assessment of chemotherapy toxicity (e.g., CRASH Score, CARG-TT).
    • Estimation of (noncancer) life expectancy.
    • Functional assessment (e.g., instrumental activities of daily living).
    • Comorbidity review (e.g., CIRS-G).
    • Fall risk screening (e.g., Get Up and Go test, BBS).
    • Depression screening (e.g., GDS-15).
    • Cognitive impairment screening (e.g., Mini-Cog, MoCA).
    • Nutritional status assessment (e.g., weight loss, BMI).
  • Please see the ASCO guidelines for additional recommendations and potential guided interventions.

Score >14: Consider proceeding with planned oncologic treatments, but remain vigilant for signs of emerging frailty or functional decline.
