2012 ACR/EULAR Classification Criteria for Polymyalgia Rheumatica (PMR)

Classifies polymyalgia rheumatica by expert consensus recommendations.

This tool should only be used in patients who: are ≥50 years of age, present with bilateral shoulder pain, and who have an abnormal ESR and/or CRP.

Morning stiffness duration

Hip pain or limited range of motion

RF or ACPA

Other joint pain

Ultrasound criteria (optional)

At least one shoulder with subdeltoid bursitis and/or biceps tenosynovitis and/or glenohumeral synovitis (either posterior or axillary) and at least one hip with synovitis and/or trochanteric bursitis

Both shoulders with subdeltoid bursitis, biceps tenosynovitis or glenohumeral synovitis

Advice
  • Patients scoring ≥4 without ultrasound are classified as having PMR.
  • Patients scoring ≥5 with ultrasound are classified as having PMR.
  • Initial therapy usually includes oral steroids but clinical judgment should be used when determining treatment pathway.
