Screens for undiagnosed type 2 diabetes mellitus and prediabetes.

Waist circumference at the level of the belly button

Measure after breathing out.

Physical activity such as brisk walking for at least 30 minutes each day

How often do you eat vegetables or fruits?

History of high blood pressure OR prescription for high blood pressure

History of high blood sugar either from a blood test, during an illness, or during pregnancy

Given birth to a large baby weighing 9 pounds (4.1 kg) or more

Family history of diabetes

Do not count multiple children or siblings twice.

Ethnic groups of biological (blood) parents

If more than one group applies, choose the one with the highest score.

Routine screening for type 2 diabetes is not recommended.

High

Hemoglobin A1c screening every 3–5 years.

Very high

Hemoglobin A1c screening annually.
