Aldrete Score
Assesses readiness for discharge from post-anesthesia care unit (PACU).
This tool includes options for:
- Modified Aldrete Score (MAS).
- Expanded Postanesthetic Recovery Score for Patients Having Anesthesia on Ambulatory Basis (PARSAP).
Pain, nausea/vomiting, temperature, bleeding, and surgical-site issues are not directly assessed by the 5-item score; evaluate them separately and escalate monitoring or level of care as indicated.
Thresholds may vary by institution; refer to your local protocols.
Inpatient surgery:
- Score ≥9 and clinically stable: Transition to Phase II or an inpatient unit per facility protocol.
- Score <9 (or any category with a score of 0):
- Identify the cause (e.g., hypoxia, hypotension, somnolence, inadequate motor function).
- Intervene: airway support, titrate O₂, treat hypoventilation (stimulate, reverse agents if indicated), fluid/vasoactive therapy for hypotension, manage pain/antiemetics.
- Continue PACU-level monitoring and reassess at intervals until criteria met; escalate to anesthesia if deficits persist.
Ambulatory/same-day surgery:

If the patient does not meet fast-tracking criteria (per local protocol):

Proceed with Phase I recovery in PACU.

Transition to Phase II when MAS ≥9 and clinically stable (similar to above guidance for inpatient surgery).

Once PARSAP ≥18 in Phase II:

Appropriate for discharge home.

Ensure pain control, minimal nausea and vomiting, and safe ambulation.
