Estimates functional capacity.

Answers are self-reported. Provides an estimate of functional capacity, and may not be as accurate as objective measurements, such as exercise stress testing.

Is the patient able to:

Take care of self

e.g. eating, dressing, bathing, using the toilet
Walk indoors

Walk 1–2 blocks on level ground

Climb a flight of stairs or walk up a hill
Run a short distance

Do light work around the house

e.g. dusting, washing dishes

Do moderate work around the house

e.g. vacuuming, sweeping floors, carrying in groceries
Do heavy work around the house
e.g. scrubbing floors, lifting or moving heavy furniture
Do yardwork
e.g. raking leaves, weeding, pushing a power mower

Participate in moderate recreational activities
e.g. golf, bowling, dancing, doubles tennis, throwing a baseball or football

Advice
  • Scores are correlated with VO2 max and metabolic equivalents (METs), which are typically measured through cardiopulmonary exercise testing (CPET).
  • Higher scores indicate better functional capacity.
  • In preoperative assessments:
    • A score ≥34 suggests a lower perioperative risk.
    • A score <34 is associated with an increased perioperative risk.
    • Functional capacity is an important part of preoperative cardiac risk assessment, but additional patient factors such as comorbidities, type of surgery, and symptoms should also be evaluated.
