Clinical Dementia Rating (CDR) Scale
Stages dementia, including Alzheimer disease, in elderly patients.
This tool provides two results.
CDR-Global is an algorithmic approach which is illustrated in the Evidence tab.
CDR-SB is the straight addition of the point selections in each category.

Use the results to guide care plans, including the need for caregiver support, home health services, or long-term care options.

Discuss the results with family members to provide them with a clear understanding of the patient’s needs and anticipated changes.

For AD specifically, severity can guide pharmacologic strategies:

Mild to moderate: Consider a trial of a cholinesterase inhibitor.

Moderate to severe: The addition of memantine may be appropriate.

Patients with early AD may be referred to a neurologist or dementia specialist for consideration of amyloid-targeting therapies.

Reassess periodically to monitor disease progression, as scores may change over time.
