Multiple Myeloma Response Criteria

Predicts disease response in multiple myeloma, from the International Myeloma Working Group (IMWG).

Response criteria apply only to patients with measurable disease, defined as ≥1 of the following:

  • Serum M-protein ≥1 g/dL.
  • Urine M-protein ≥200 mg/24 hr.
  • Serum FLC level ≥ 10 mg/dL with an abnormal FLC ratio.

Patients who do not meet any of the measurable disease criteria above may only be assessed for stringent complete response (sCR).

See Evidence for criteria if serum and urine M protein are unmeasurable

25% increase from lowest response value in the following (select any that apply):

Absolute increase must be ≥0.5 g/100 mL
Absolute increase must be ≥200 mg per 24 hrs
Absolute increase must be >100 mg/L
Absolute % must be ≥10%
Corrected serum calcium must be >11.5 mg/dL

Advice
  • Results are primarily used in clinical trials to monitor treatment response.
  • While no specific treatment recommendations are directly tied to results, response assessment can help determine whether a patient is meeting treatment goals and inform potential therapy adjustments.
